Our Client is seeking motivated and reliable professionals to work as Care Coordinators in the Fort Worth, TX area.
HOURS Full-time-all shifts SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITYThe Care Coordinators participates in observing, reporting, and recording of client behaviors under the supervision of the Lead Care Coordinator.
Responsible for helping to provide a safe and secure environment and therapeutic milieu for clients and staff in compliance with hospital policies and procedures.
DUTIES & RESPONSIBILITIES Attending bi-weekly trainings; Documentation of observations:
To observe, report, and record client's behavior.
Consistently demonstrates understanding of assigned procedures and implements according to nursing policy manual Aware and accountable for each assigned client and activities Consistently reports client behavior to Clinical/Nursing staff.
Quickly anticipates and interprets client behavior to prevent crisis situation.
Consistently documents client's behavior in the medical records, per hospital procedure.
Assists Therapy Staff with groups and activities as assigned by Clinical Director or Director Assists in maintaining a safe, therapeutic environment that maintains client's rights.
Demonstrates understanding of therapeutic Clinical/Program.
Sets limits on client's behaviors as needed to uphold the rules and regulations of the Latency/Adolescent/MBR/Autism Clinical/Programs and the hospital.
Assists the RN or LVN in assessing the need for seclusion, restraints, or suicide/elopement precautions and implements such measures in conformity with hospital policy.
Assists with the management of combative or self-destructive clients, using approved techniques.
Conducts groups as assigned by the Clinical/Program Director including but not limited to Current Events, Health and Hygiene.
Assists in the obtaining vital signs as ordered, collecting specimens for testing as needed, as well as Blood-Sugars on glucometer; assists RN or LVN with special treatments as needed.
Assists in transporting and escorting of clients as needed.
Assists with the various clerical functions required to assure efficient unit operation.
Carries out routine security checks of the facility.
Pursues increased clinical competence through independent reading and/or class work.
Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the clients served on his or her assigned unit.
CLIENT'S REQUIRED EXPERIENCE & EDUCATION High School Diploma or GED.
Bachelor's degree in a human or social service-related field preferred Must be able to clear a background check Current negative TB test, drug test Must be at least 21 years of age At least 1 year relevant experience working with children and/or adolescents LICENSURE/CERTIFICATIONS Must participate in annual behavior management and physical restraint training CPR/First Aide Certification DTG ADVANTAGES Establish a relationship with one of the nation's largest referral agencies for behavioral-health.
